Insurance Information

It is your responsibility to keep us updated with your correct insurance information. If the insurance company you designated is incorrect, you will be responsible for payment of the visit and to submit the charges to the correct plan for reimbursement. It is your responsibility to understand your benefit plan regarding, for instance, covered services and participating laboratories. 


For example:

  • Some plans may not cover mental health services. If your insurance does not cover mental health services, you will be responsible for payment.
  • Certain insurance companies may limit the number of allowable well mental health visits per year. If the number of visits is exceeded, your insurance company will not pay; you will be responsible for payment. 


It is your responsibility to know if a written referral or authorization is required before your visit, and what services are covered. 

Referrals

  • We are not a crisis center. Urgent/emergent referrals will not be accepted.
  • MOYA Mental Health Services provides outpatient treatment/medication management, and is not a crisis center and does not provide emergency services. If you or someone you know is experience a psychiatric or mental health emergency, please call 911 or go to the nearest hospital emergency room.
  • All efforts will be made to schedule non-emergent referrals within 5 business days if there is an open appointment. 
  • It is your responsibility to check with your insurance/confirm if we participate in your plan.

Release/Transfer of Records

  • If you transfer to another practice/physician, we can provide a copy of your last visit notes to your physician, free of charge, as a courtesy to you. We need 48 hours’ notice.
  • For any previous records, you must request them directly from your previous doctor(s).

Prescription Refills

  • For monthly medication refills, we require 48 hours’ notice, during regular business hours. Please plan accordingly.

Financial Responsibility

  • You are responsible for any co-payments, deductibles, and coinsurances processed through our billing company
  • Self-pay patients are expected to pay for services in FULL at the time of the visit. 
  • If we do not participate in your insurance plan, payment in full is expected from you at the time of your visit. We will supply you with an invoice that you can submit to your insurance for partial reimbursement. We do not assist with reimbursements from your out of network insurance. It is your responsibility to contact and submit your invoice to your out of network insurance. Please contact your insurance company first before you consider coming to an out of network provider. 
  • If previous arrangements have not been made with us, any account balance outstanding longer than 60 days will be charged a $25 fee for each 30-day cycle. Any balance outstanding longer than 90 days will be forwarded to a collection agency. For scheduled appointments, prior balances must be paid prior to the visit. Overdue accounts will be referred to a collection agency as a last resort. AS MOYA Mental Health Services, we will try our best to work with our clients and we assume our clients will do the same. We accept branded cards only (Visa, MasterCard, Discover, AmEx, etc.). You will be required to leave a confidential credit card on file that will be billed for services provided and not covered by your insurance. Your credit card will also be used to secure payment in case of repeated no-show/late cancellations or outstanding fees and balances. We do not accept alternate payment types such as Apple Pay, Google Pay, Cash Apps or Venmo.

Payment for Services

Please note that you are ultimately responsible for all charges incurred for your treatment or the treatment of those for whom you are responsible. If for any reason your insurance company, or other third-party payer (such as a divorced spouse), does not promptly reimburse MOYA Mental Health for services rendered, you will be responsible for those charges.


You will be required to leave a confidential credit card on file that will be billed for services provided and not covered by your insurance. HSA and FSA cannot be put on file but can still be used as you go if you are certain you have funds in them. Your credit card will also be used to secure payment in case of a no show and late appointment cancellation and other services not covered by insurance.

Laboratory Studies

Lab work may be required prior to initiating or continuing medication. If laboratory studies are required you will be referred to an outside laboratory company Sonora Quest. The provider will order the labs. Please be aware that the cost of lab work done by laboratory companies is not included in your visit charge for MOYA Mental Health Services and it is your responsibility to pay for lab work.
